hCG Cost: $171.51 for a 10,000-Unit Pregnyl Vial, the Same Price for Half as Much Novarel, and What the Label's Own Regimens Add Up To

What hCG (chorionic gonadotropin) costs, from the national pharmacy acquisition cost Medicaid publishes, read on 3 November 2026: Pregnyl 10,000 units $171.51, Novarel 5,000 units $171.65, generic 10,000 units $252.90 and the Ovidrel prefilled pen $242.78. Converted to price per 1,000 units, with the label regimens costed and the 30-day and 60-day discard rules that decide how much of a vial is used.

hCG is sold in the US as four FDA-licensed products, all prescription only: Pregnyl (Organon) and Novarel (Ferring) — urinary hCG measured in USP units — a generic chorionic gonadotropin vial, and Ovidrel (EMD Serono), recombinant choriogonadotropin alfa in a prefilled syringe measured in micrograms. What hCG does and does not do in the body is on our hCG side effects page; how it compares with menotropins is on hCG vs hMG.

This is a dated price survey, method as for every cost page on this site: published figures only, from the source that collects them, with the date read. The national price-comparison sites refused an automated read on 3 November 2026 and are excluded, not quoted from snippets. What remains is NADAC — the National Average Drug Acquisition Cost, Medicaid's weekly survey of what US retail pharmacies paid wholesalers.

The four products, per vial and per 1,000 units

Product Pack NADAC per pack (as of 21 Oct 2026) Per 1,000 units
Pregnyl 10,000 USP units, multi-dose vial $171.51 $17.15
Generic chorionic gonadotropin (Fresenius Kabi) 10,000 USP units, vial $252.90 $25.29
Novarel 5,000 USP units, multi-dose vial $171.65 $34.33
Ovidrel 250 mcg / 0.5 mL prefilled syringe $242.78 not comparable (micrograms)

Per-1,000-unit figures and the Ovidrel syringe price ($485.56 per mL × 0.5 mL) are our arithmetic. NADAC held no row for a 10,000-unit Novarel vial on 3 November 2026.

Two things stand out. The generic costs more than the brand — $252.90 against $171.51 for the same 10,000 units, 47% more. And Novarel 5,000 and Pregnyl 10,000 cost the same per vial, so per unit Novarel is twice the price.

The price has not moved

Pregnyl 10,000 units across NADAC's 2025 and 2026 files:

NADAC effective date Per vial
18 December 2024 $171.80
23 July 2025 $171.60
21 January 2026 $171.78
21 October 2026 $171.51

A 0.2% drift in two years. For comparison, a brand peptide drug on this site, Linzess, saw its NADAC halve overnight on 1 January 2026 (our Linzess cost page).

What the label's regimens add up to

The Novarel label lists dosing regimens "advocated by various authorities" and states that the regimen in any case depends on the indication, age, weight and the physician's preference. Costing those regimens is arithmetic on the label's text; it is not a dosing suggestion, and which regimen applies is the prescriber's decision.

Label regimen (Novarel label, section 2) Total units Pregnyl 10,000 vials At Pregnyl NADAC
Ovulation induction after menotropins: 5,000–10,000 units once 5,000–10,000 1 $171.51
Hypogonadotropic hypogonadism, short course: 500–1,000 units 3× weekly for 3 weeks, then 2× weekly for 3 weeks 7,500–15,000 1–2 $171.51–$343.02
Hypogonadotropic hypogonadism, long course: 4,000 units 3× weekly 48,000 per 4 weeks 5 per 4 weeks $857.55 per 4 weeks
Prepubertal cryptorchidism: 4,000 units 3× weekly for 3 weeks 36,000 4 $686.04

Whole vials are counted because a vial is sold whole. The long hypogonadism regimen in the label runs six to nine months at that dose, then reduces; six months at 4,000 units three times a week is about 312,000 units, or 32 vials and $5,488 at Pregnyl's acquisition cost.

The discard rule changes the count

Product After mixing (label)
Pregnyl Refrigerate; discard after 60 days
Novarel Refrigerate; use within 30 days

The short hypogonadism course above runs six weeks. At its lower dose (7,500 units) it fits in one Pregnyl vial within the 60-day window. With Novarel 5,000 it needs two vials on units alone — and a single Novarel vial would pass its 30-day limit before the course ends anyway. The cheaper unit price and the longer shelf life after mixing point the same way here.

What these figures do not tell anyone

  • A counter price. NADAC is what pharmacies paid; the price a patient is charged adds a margin, and fertility pharmacies set their own.
  • Insurance. Fertility coverage varies by state mandate and plan; nothing here describes a copay.
  • Compounded or "research" hCG. Since 23 March 2020 hCG has been a licensed biological product without the compounding exemptions (the transition is set out on our hCG side effects page). Prices for unlicensed products are not surveyed here.
  • Weight loss. Every hCG label states it has not been shown to be effective for obesity; see the hCG diet.

The men's prescribing record is on hCG for men; prices across the wider peptide market are on how much do peptides cost.

Frequently asked questions

How much does hCG cost?

At the national average price pharmacies paid in October 2026, one 10,000-unit vial of Pregnyl cost $171.51, a 5,000-unit Novarel vial $171.65, a 10,000-unit generic vial $252.90 and one Ovidrel 250 mcg prefilled syringe $242.78. What a patient pays adds the pharmacy's margin and depends on insurance; no retail cash price could be read for this survey.

Which hCG is cheapest per unit?

Pregnyl, on this data: $17.15 per 1,000 units, against $25.29 for the generic 10,000-unit vial and $34.33 for Novarel 5,000. Ovidrel is measured in micrograms, not USP units, so it cannot be compared per unit; it is sold as a single pre-measured dose.

How much does a course of hCG cost?

It depends entirely on the indication and the regimen the prescriber chooses. Costed at Pregnyl's acquisition price, a single 10,000-unit dose after menotropins (a regimen the label lists for ovulation induction) is one vial, $171.51. The label's longest regimen for male hypogonadotropic hypogonadism, 4,000 units three times a week, adds up to 48,000 units every four weeks — five vials, $857.55. These are arithmetic on label text, not a recommendation.

Can hCG be bought from a compounding pharmacy?

Not from bulk hCG since 23 March 2020, when hCG moved to regulation as a licensed biological product and lost the exemptions that let pharmacies compound it. The FDA-approved products are Pregnyl, Novarel, the generic chorionic gonadotropin vial and Ovidrel.

Are hCG diet drops real hCG?

The hCG products FDA licenses are prescription injections. The labels state that hCG has not been shown to be effective in treating obesity. Over-the-counter 'hCG' drops are a different category; the record on them is on our hCG diet page.
